Jan 26th 2008 2:19PM The rebate form is almost impossible to find but here it is I think:

Sears says it is not their rebate but a manufactures rebate and the form is in the box. HA, not only is there no form, there is no address or phone number for Mirus!
I hope I get my rebate.
