Hmm, I think an old co-worker had a worse one...using the typical "first initial plus last name" conventions, the company sought fit to give my coworker the following e-mail address:
P (first initial) + Ennis (last name) = Pennis@company.com
And this is no lie...
...finally, after about a year of having this e-mail, the company finally changed it to first name plus last initial...

When I worked at a midwest retailer, the computer guys wanted to switch from last name, first initial to first initial, last name for email addresses. Seemed like a huge waste of time, money and effort to me. I was in charge of the data warehouse and submitted a new user request for Dave Ickless. I got a very serious call from the manager saying they couldn't give that name out.... We laughed for months after that!
